注意是我们认识世界的基本机制,也是对我们思想和感受的主动调节,它的产生取决于外部刺激的特点和人的主观因素。对儿童的注意力评价极其重要,因为它对儿童的学习能力、学业成就和社交能力等具有重要影响。 对注意本身的研究一般采用视觉搜索范式,即在一系列刺激中,让被试寻找出目标刺激。行为实验的研究结果认为,随着刺激序列中物体数量的增加,搜寻目标刺激所用的时间就越长。一种观点认为这是系列加工的结果,另一种则认为是由于平行加工的容量有限,从而导致随着刺激的增多,找出目标的时间增加。简单的行为实验并不能解释这两种观点的矛盾,因为它不能测得每一时刻的空间注意转换。随着功能性磁共振成像(fMRI)技术的出现,人们对注意有了更深入的研究。波斯纳(Michael Posner)等人于1990年提出了注意的网络模型,认为注意网络包含三个子系统,即觉醒、定向和执行控制。随着更深入的fMRI研究,研究者将定向注意系统又分为背侧注意网络(dorsal attention network,DAN)和腹侧注意网络系统(ventral attention network,VAN)。DAN包含额眼区(frontal eye fields,FEF)、顶内沟(intra-parietal sulcus,IPS)和视觉区(visual area,VA)等,主要负责对当前刺激产生内源性定向注意,负责视觉空间注意和自上而下型(top-down)加工。VAN主要包含颞—顶联合区(temporal parietal junction,TPJ)以及腹侧额叶皮层中的额中回(middle frontal gyrus,MFG)、额下回(inferior frontal gyrus,IFG)和岛叶(insular lobe)等。VAN通常在对那些突出的和不被注意的事件的侦测过程中被征用,主要参与刺激驱动的自下而上型(bottom-up)注意。大量研究表明,与注意有关的神经结构是在人出生后,经过长时间持续发展的,这些发展至少持续到3岁。在婴儿期,觉醒注意和定向注意开始发展;在1岁左右,自主控制的注意逐渐兴起;在童年期,三种注意网络持续发展,其中执行注意网络的发展依赖于前额叶的成熟,因此要持续到青春期,而对选择性注意的征用和控制能力的发展则会持续到成年早期。例如,对于青少年,背景噪音会产生更大的干扰效应,并且他们在侧抑制(flankers)任务中相对成年人也表现出较大的flankers效应。虽然儿童一般情况下选择性注意能力较弱,但是在某些条件下却能够产生和成年人一样的效果。例如,如果在目标呈现之前,采用一个空间预先提示(pre-cue)线索吸引儿童的注意,那么儿童的注意能力和成人几乎一样。这意味着,即使在年幼儿童中,如果具有足够的线索来提供直接选择性注意,那么选择性注意就能够被征用。除此之外,当注意要求较高的时候,儿童和成年人能够表现出等量的flankers效应。这一现象被解释为注意知觉负荷理论,即当注意需求增加时(比如知觉负荷变大),大多数可以利用的注意资源都被投入到目标刺激中,因此只剩下寥寥可数的注意资源投入到干扰刺激的加工中,所以导致儿童flankers效应的减少。除此之外,儿童处理反应冲突的能力可能发展比较缓慢,也就是说注意系统中的执行控制网络发展相对缓慢。该网络涉及监测和解决思想、感觉和反应之间的冲突,是有效规范行为能力的核心。这个网络系统主要与前扣带回(anterior cingulate cortex,ACC)、外侧前额皮层(prefrontal cortex, PFC)以及多巴胺系统有关。婴儿期部分执行网络得到发展,成人与婴儿在感知错误激活的区域相似,比如ACC。但是,证据表明,这些区域直到4岁才会与运动输出联结或者是控制运动输出。在此之前,主要是通过定向网络进行控制。对4—21岁的750个被试的fMRI研究发现,童年早期,右半球ACC的大小最能够预测解决冲突。另外,采用变化色—词的斯特鲁普(stroop)任务,其研究结果表明,较年轻的儿童相对于成人,在前额叶征用中表现出大约2秒的滞后。证据表明,在stroop任务中随着年龄增加,顶叶征用更多。而在stroop任务中ACC发展的年龄变化在现有研究中尚未达成共识,有的观点认为ACC的激活随着年龄增加而减少,有的观点则认为反之。有研究单独采用flankers任务,在儿童和成人中探究了相对于中性条件下的神经活动,发现8—12岁的儿童相对于成年人表现出前额叶的不对称征用。尤其是,儿童表现出较大的左侧PFC激活,而成年人表现出更大的右侧PFC激活。这些差异可能反映了注意策略的可能变化。一些生理上的疾病,比如自闭症谱系障碍(autism spectrum disorder,ASD)和注意缺陷多动障碍(attention deficit hyperactivity disorder,ADHD)群体,在注意网络的发展上与正常人存在较大差异。虽然注意缺失并不是ASD的关键特征,但一些研究者提出注意障碍可能与ASD表征的问题表现有关。因为注意加工是执行复杂认知动作的一个重要成分,早期注意缺失的出现可能会干扰更高水平认知和社会技能(比如执行功能和联结注意)的发展。反过来,这可能会导致ASD的典型行为问题,比如应对变化的困难,关注环境细节的倾向,以及从事重复和固定行为的倾向。对ASD的弥散张量成像(diffusion tensor imaging,DTI)研究发现,和正常被试相比,ASD被试脑中长纤维的连通性减少,尤其是在胼胝体和钩状束上。而另一个研究发现自闭症个体在儿童时期表现出定向网络的超连接性,但是在成年中表现为弱连接性。此外,相对于正常发展的婴儿,ASD风险婴儿在出生后前6个月内,在观看一个人讲话时,其放在说话者眼睛上的注意力明显减少。而被诊断为ASD的婴儿在观看一张人脸的时候,在3—9个月的时候表现出一个相反的模式(对眼睛的注意增加)。另外,在将ASD儿童和青年同正常被试进行比较时,基恩(Brandon Keehn)等人发现,在各个注意网络的效率上,ASD儿童只存在定向网络受损。另一种与注意网络发展有关的神经精神疾病被称为ADHD,它表现为与发育水平不相称的注意力不集中、冲动和多动,这会对儿童生活的各个领域产生消极影响,其中包括建立健康的人际关系能力。ADHD患者对他人的眼睛注意敏感性较差,这与右半球的功能障碍活动有关,该大脑区域主要参与人眼线索的定向注意。此外,其他脑区也涉及注意缺陷多动障碍,包括人脸知觉的颞叶区域(梭状回和颞上回),额顶叶注意区域和情绪社会认知的前额叶区域。在利用注意网络测试(attention network test,ANT)对ADHD儿童的三种注意网络进行测试时,约翰逊(Katherine A. Johnson)等人发现,ADHD儿童在觉醒注意网络和执行控制注意网络上存在缺失,主要表现在ADHD儿童的觉醒水平较低,并且ADHD儿童更容易受到干扰。伊丽莎白(Elisabet Suades)等人采用ANT测试对7—11岁的2000多名儿童(其中10%具有ADHD症状)进行了纵向研究,结果也表明ADHD儿童的定向注意网络随着年龄并没有发生显著性变化。另外一些研究表明ADHD儿童存在社会线索加工缺失,对社会信息有偏爱,他们比起正常儿童会产生更多不合适的社会反应。马洛塔(Andrea Marotta)等人采用线索范式发现,ADHD儿童仅仅对非社会刺激线索(箭头和边缘线索)产生定向反射,未能对社会注视线索的反应提供定向反射。这些发现认为注视方向的定向加工在ADHD儿童中可能受损。 目前,对这些特殊儿童,除了进行药物控制矫治以外,主要依靠大脑的可塑性对其进行认知训练,从而提高大脑网络结构的功能来调节注意缺陷的症状。到目前为止,针对ADHD儿童,主要采用的是父母训练方法,即根据社会学习理论教给父母相应的技术,以期他们能够纠正孩子的行为表现,另一种方法则是直接对患者进行认知训练,这种认知训练主要针对执行控制功能,但是两者的效果均不是很理想。因此在后续的研究中,针对这些特殊儿童,我们还应该另辟蹊径进行矫治,使得他们在今后的生活中能够顺利成长。
